  24. MED_ICD9 System requirements:
  25.         MS-DOS 3.30 (or greater) (DOS 5.0 preferred)
  26.         640k of memory (required)
  27.         Monochrome (or color) monitor
  28.         (1) 5 1/4" floppy disk drive (for loading of software)
  29.         (1) hard disk drive (for program and data files)
  30.         (1) printer (any dot matrix printer)
  31.  
  32. Files contained on your distribution disks:
  33.      README.1ST       : Basic instructions and current notes
  34.      MED_ICD9.EXE     : The 'MED_ICD9' program
  35.      MAKELOK.COM      : Program required for network users
  36.      ICD9TEMP.DBF     : Sample ICD-9-CM code table data file
  37.      PRTMANL.BAT      : Routine to print user manual
  38.      MED_ICD9.DOC     : User manual

  41. Steps to follow for system installation:
  42. ---------------------------------------
  43. 1.  Make a backup of the MED_ICD9 distribution disk and store the 
  44. original in a save place.  Use the newly created backup disk  for 
  45. all following installation instructions.
  46.  
  47. 2. Check to insure that you have a 'CONFIG.SYS' file in your root 
  48. directory with the following MINIMUM values:
  49.  
  50.            FILES = 20 
  51.            BUFFERS = 9
  52.  
  53. If  you do not have a 'CONFIG.SYS' file, or it does  not  contain 
  54. these  minimum  values,  consult  your  DOS  manual  on  how   to 
  55. create/modify  your 'CONFIG.SYS' file. (Note - MED_ICD9 will  not 
  56. operate  properly, and may crash, if your 'CONFIG.SYS' file  does 
  57. not exist with these values. If you needed to create, or  modify, 
  58. your  'CONFIG.SYS'  file  be sure that you   reboot  your  system 
  59. before you continue. Your system only reads the values  contained 
  60. in your 'CONFIG.SYS' file at initial system boot.)
  61.  
  62. 3.  If  you do not have a MED1 sub-directory now is the  time  to 
  63. make one.  This is done by entering the command:
  64.  
  65.                         C>MD\MED1 <enter> 
  66.  
  67. 4.  Make  your  MED1 sub-directory  your  current  default  sub-
  68. directory.   
  69.  
  70.                         C>CD\MED1 <enter>
  71.  
  72. 5.   Insert   MED_ICD9 distribution disk into drive   'A'.   Copy  
  73. the  contents  of  drive  'A'  into the  your   current   default   
  74. sub-directory by entering the following DOS command:
  75.  
  76.                       C>COPY A:*.* <enter>
  77.  
  78. Remove disk from drive 'A' upon completion.
  79.  
  80. 6. Print your user manual and read before going any further:
  81.                 a: turn on your printer
  82.                 b: enter C>PRTMANL  <return>
  83.  
  84.  
  85. 7. PEER TO PEER NETWORK USERS:  After you have loaded all of  the 
  86. distribution files onto your hard disk you will need to create  a 
  87. small  file  named 'PCNETLIB.LOK' used by your network  to  track 
  88. file status.  The 'MAKELOK.COM' program will create this file for 
  89. you.  To execute the 'MAKELOK.COM' program just enter:
  90.  
  91.                         C>MAKELOK <enter>
  92.  
  93. This 'PCNETLIB.LOK' file must reside in the same sub-directory as 
  94. your 'MED_ICD9' data files.
  95.  
  96.  
  97. ALL    COPIES    OF    'MED_ICD9'  WILL   INITIALLY    LOAD    AS  
  98. 'UNREGISTERED'.  WHILE IN THIS 'UNREGISTERED' MODE YOU WILL  HAVE 
  99. TWO  (AND  LATER  THREE)  15  SECOND  PRODUCT  PROMOTION  SCREENS 
  100. DISPLAYED  TO  YOU  WHEN YOU EACH TIME YOU  LOAD  THE  'MED_ICD9' 
  101. PROGRAM.   THIS  WILL  CONTINUE  UNTIL YOU  ENTER  A  VALID  USER 
  102. REGISTRATION  NUMBER. THIS 'USER REGISTRATION  NUMBER' IS  UNIQUE 
  103. TO  EACH USER AND IS  PROVIDED  TO THE  USER  UPON RECEIPT  OF  A 
  104. FULLY PAID AND PROPERLY  FILLED  OUT REGISTRATION FORM.

  107. REGISTERED USERS WILL RECEIVE THE FOLLOWING BENEFITS:
  108.         >FREE BUG REPLACEMENT DISKS;
  109.         >PRINTED USER MANUAL;
  110.         >CURRENT VERSION OF MED_ICDI;

  113. REGISTERING YOUR COPY OF MED_ICD9:
  114. ---------------------------------
  115.      After   you've  worked with the  un-registered   package  of 
  116. MED_ICD9,   and  decide  you want full  access  to  the   system,  
  117. follow these simple steps to register:
  118.  
  119.      1.  Enter the 'MED_ICD9 UTILITIES'
  120.      2.  Verify your personalization information
  121.      3.  Select 'PRINT REGISTRATION FORM'
  122.      4.  Fill out any missing information
  123.      5.  Send in registration form and required registration
  124.          fee to MEDshare.

  132.            *** COMMON PROBLEMS AND THEIR SOLUTION ***
  133.            ******************************************
  134.  
  135. 1.  INSUFFICIENT  MEMORY: MED_ICD9 operates  totally  within  the  
  136. 640K  limits   of  your system. This area is also  used  by  your  
  137. operating  system and other programs you may have been using.  To 
  138. see  if   you have sufficient memory to  load MED_ICD9  run   the  
  139. DOS   'CHKDSK' routine.  You should have in excess of 512 K  free  
  140. memory.  Next, after you have loaded MED_ICD9 press the 'F10' key 
  141. when  you are at the DATE PROMPT on the MED_ICD9  sign-n  screen.  
  142. You   should    have  available   free  memory  of  100K.  (NOTE: 
  143. MED_ICD9 REQUIRES UP TO  AN ADDITIONAL 100K OF MEMORY FOR SEVERAL 
  144. OF  ITS  FEATURES TO RUN.  IF YOU DO NOT HAVE THIS 100K  OF  FREE 
  145. MEMORY  CHECK YOUR  'CONFIG.SYS' AND  'AUTOEXEC.BAT'   FILES   TO 
  146. SEE WHAT OTHER  PROGRAMS  YOU  ARE USING.  THE  BEST  TEST  IS TO 
  147. REMOVE  ALL  BUT  THE  'FILES'  AND 'BUFFER' COMMANDS FROM  THESE 
  148. FILES,  REBOOT,  LOAD MED_ICD9, AND SEE HOW MUCH FREE  MEMORY  IS 
  149. AVAILABLE. THEN ADD BACK IN THE  FEATURES YOU WANT UNTIL YOU LOSE 
  150. THE 100K REQUIREMENT.)
  151.  
  152. 2. ERROR MESSAGES ENDING WITH A '(2)': This error is a result  of 
  153. MED_ICD9  not being able to locate its data files.   This   error  
  154. is normally  a result of MED_ICD9 being installed on one   system  
  155. and  then  copied   to  another  system  and  to   a    different   
  156. disk  drive  designation.   To   correct   this   problem    just   
  157. delete  the  'ICDMVBLS.MEM' file then re-load MED_ICD9. You  will 
  158. be  taken into the  system initialization cycle.

  160. 3.  ERROR MESSAGES ENDING WITH A '(4)' or 'DATA  BASE  REQUIRED': 
  161. This error is a result  of MED_ICD9 not being able to open a file 
  162. due  to  limits  controlled by your    DOS    system.   This   is  
  163. corrected   by   correcting   your 'CONFIG.SYS'  file   to   have 
  164. the  minimum  'FILES'  and  'BUFFERS' commands  as  required  for 
  165. system  setup. After you have  corrected your  'CONFIG.SYS'  file 
  166. reboot your system - THIS IS THE ONLY TIME WHAT THESE  PARAMETERS 
  167. ARE READ.
